If you are going to be a guest at Lena's Adult Day Services, we ask that you please fill out the following document.

Note: These are all the same documents, we provide several varieties so that you can use the appropriate document depending on what software you have.

Caregiver Checklist

chklst.pdf (Adobe Acrobat PDF)
chklst.xls (Microsoft Excel)
chklst.ods (OpenDocument Spreadsheet)

File Types

Adobe PDF files:

PDF files are opened with Adobe's Acrobat software. If you would like to open these files, you can download the free Acrobat Reader Software.

Microsoft Excel files:

Microsoft Excel .xls files are opened using Microsoft Office. You can find more information about Microsoft Office and related products at the Office homepage.

OpenDocument files:

Files with the open document format can be opened by any application that chooses to support the open document format. One of these free applications is OpenOffice.org.
